Position SummaryThe organization is seeking a dedicated and compassionate Nurse Practitioner (NP) to join its full-time nursing home and house call team in Brooklyn, NY.
This is a full-time, outpatient-focused role providing medical care to patients in skilled nursing facilities and home-based settings. The Nurse Practitioner will manage acute and chronic conditions for elderly and medically complex patients, with a strong focus on continuity of care, preventive services, and reducing unnecessary hospitalizations.

Essential Position Responsibilities- Provide comprehensive medical care to patients in nursing homes and home-based settings
- Perform history and physical examinations, routine follow-ups, and urgent evaluations
- Diagnose and manage acute and chronic medical conditions
- Order, interpret, and evaluate diagnostic tests and labs
- Develop and manage individualized treatment and care plans
- Coordinate care with primary care physicians, specialists, nursing staff, and caregivers
- Educate patients and families on disease management, medications, and preventive care
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Participate in care coordination, quality improvement initiatives, and population health efforts
- Education:
Graduate of an accredited Nurse Practitioner program (MSN or DNP) - Licensure & Certification:
Active or eligible New York State Nurse Practitioner license; national certification required (FNP, AGNP, or GNP preferred) - Experience:
Prior experience in nursing home, house call, geriatrics, or primary care preferred; new graduates are welcome to apply
