Registered Nurse (RN)

The Registered Nurse (RN) provides skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care. This role involves conducting assessments, developing care plans, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory guidelines.

Key Responsibilities:

• Conduct comprehensive patient assessments and ongoing evaluations
• Develop, implement, and manage individualized care plans
• Perform timely home visits and ensure continuity of care
• Educate patients and caregivers on disease management and treatment plans
• Maintain accurate and compliant clinical documentation (DOH, OMIG, etc.)
• Monitor patient progress and recommend care plan adjustments
• Respond to emergencies and changes in patient condition
• Supervise and guide Home Health Aides
• Coordinate care with physicians, therapists, and healthcare teams
• Participate in quality improvement initiatives

Required Qualifications:

• Active RN License - New York
• Bilingual (English/Spanish) required
• Minimum 2-3 years of clinical experience
• Strong knowledge of home health practices
• Valid driver's license and willingness to travel for home visits

Preferred Qualifications:

• Experience in home health or certified home health agencies
• Familiarity with DOH, HRA, MCO, MLTC guidelines
• Strong wound care, chronic disease management, and patient education skills

Work Environment:

• Field-based role (patient homes)
• Local travel required
• Independent work with team collaboration
