Clinical Assessment Nurse (RN) – UAS-NY
Location: Bronx & Manhattan, NY (Local Travel Required)
Pay: $62–$65/hour

• Full-Time

• Conduct UAS-NY Initial Assessments, Reassessments, and Comprehensive Clinical Evaluations in members' homes.
• Complete accurate, timely clinical documentation and care plans.
• Assess physical, cognitive, psychosocial, and environmental needs.
• Collaborate with physicians, care managers, social workers, and the interdisciplinary team.
• Educate members and caregivers on available services and care recommendations.
• Ensure compliance with NYS Department of Health regulations and organizational standards.
• Travel throughout the Bronx and Manhattan to complete member assessments.

• Active New York State RN License (Required)
• UAS-NY assessment experience (Required)
• 3–5 years of experience in MLTC, PACE, managed care, home care, or a related clinical setting
• Valid driver's license and reliable vehicle required
• Strong clinical assessment, documentation, and communication skills
• Ability to travel throughout Bronx and Manhattan
