Visiting Nurse (RN Coordinator of Care) - Americare NY - New York

The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This would include conducting home visits, completion of all required documentation and communication with PCP.

Full time schedule:
Friday, Saturday, and Sunday 13 hr shifts

Requirements:

• NYS RN License
• We welcome New Grads, busy parents, retirees, hospital nurses looking for a change in pace!
• Valid driver's license and reliable transportation.
• Strong ability to work independently with minimal supervision
• Excellent clinical skills
• Basic knowledge of office applications and equipment (Outlook, Word, Printing and scanning applications).

Salary: $105,000 – $110,000 yearly

Benefits: $15K Sign on bonus
