Field Nurse (RN)

A respected healthcare organization is seeking a compassionate and dedicated Field Nurse (RN) to join its care team. Responsibilities Include:

• Perform in-home initial assessments and reassessments for patients
• Develop, implement, and revise care plans in collaboration with interdisciplinary teams
• Provide training, support, and clinical guidance to Home Health Aides and Personal Care Assistants
• Educate patients and families on care, medications, and condition management
• Complete thorough and accurate documentation
• Participate in care planning and clinical team meetings
• Ensure patient needs are met with quality, dignity, and compliance

Ideal

Qualifications:

• Active New York State Registered Nurse (RN) license
• At least 1 year of nursing experience (home care experience preferred)
• Strong communication and clinical assessment skills
• Patient-focused mindset and ability to work independently
• Reliable transportation for travel between patient homes
• Proficient in documentation and electronic medical records
