Job Summary
We are seeking an experienced Registered Nurse Manager to oversee clinical operations, care coordination, and nursing staff while ensuring high-quality, person-centered care. This role works closely with interdisciplinary teams, leadership, participants, and caregivers to promote positive outcomes, regulatory compliance, staff development, and overall patient satisfaction.
Key Responsibilities- Oversee daily clinical operations, staffing, assignments, and resource allocation.
- Develop, implement, and monitor individualized care plans to support positive outcomes.
- Lead and participate in Interdisciplinary Team (IDT) meetings and care coordination activities.
- Coordinate transitions of care, including hospital discharges, rehabilitation, nursing home, and home care services.
- Monitor clinical documentation, medical records, care plans, and quality standards for accuracy and compliance.
- Review clinical assessments and charts to identify and address deficiencies.
- Investigate and follow up on incidents, grievances, and participant concerns in accordance with regulations.
- Collaborate with providers, clinicians, facilities, caregivers, and other members of the care team.
- Supervise, coach, evaluate, and support nursing staff and other direct reports.
- Assist with interviewing, onboarding, training, performance management, and professional development.
- Promote evidence-based clinical practices and continuous quality improvement.
- Ensure compliance with organizational policies and applicable federal and New York State regulations.
- Serve as a member of the management and interdisciplinary teams and provide leadership support as needed.
Monday–Friday | 8:30 AM–5:00 PM
37.5 hours per week
- Current, active, unrestricted New York State RN license required.
- Graduate of an accredited nursing program; BSN preferred.
- Minimum 2 years of healthcare management/supervisory experience.
- Experience in home health, long-term care, acute care, medical-surgical, critical care, nursing home, or similar settings preferred.
- Minimum 1 year of experience working with frail or elderly populations preferred.
- Strong care coordination, leadership, communication, and customer service skills.
- Experience with long-term care insurance is a plus.
- Case Management Certification is preferred and may be required within two years of employment.
- Bilingual skills preferred.
- Must meet all applicable organizational, regulatory, immunization, and health requirements.
