Licensed Practical Nurse

As a Licensed Practical Nurse at Brooklyn Center, you will deliver direct, compassionate care to diverse residents in a city-run healthcare setting. Responsibilities include administering medications and treatments, monitoring patient status, documenting in EHRs, and supporting health education and prevention programs. You will collaborate with RNs, physicians, and public health teams to promote safety, wellness, and continuity of care, while upholding Brooklyn Center's values of inclusivity, accountability, and ethical, community-focused service.

Responsibilities

• Provide direct patient care, including vital signs, basic assessments, and routine treatments
• Administer medications and injections per provider orders and protocols
• Monitor, document, and report changes in patient condition using EHR systems
• Support public health and prevention initiatives, including patient education
• Assist with coordination of care, referrals, and follow-up with community resources
• Maintain infection control, safety, and quality standards in all care areas
• Collaborate with RNs, providers, and interdisciplinary teams on care plans
• Perform wound care, specimen collection, and point-of-care testing as authorized
• Educate patients and families on disease management and medications
• Uphold confidentiality, ethical standards, and cultural sensitivity with all residents
