LPN, LPN​/LVN, Palliative Care Nurse

MJHS is seeking a compassionate LPN for our Night shift to support patients across hospice, home care, and long-term care settings in the greater New York area. In this role, you will provide bedside care, administer medications, monitor and document patient status, and communicate changes to RNs and providers. You will support patients and families with dignity and respect, collaborating with an interprofessional team.

Responsibilities

• Provide overnight nursing care to patients in hospice, home care, or long-term care settings
• Administer medications and treatments according to care plans and provider orders
• Monitor, document, and report changes in patient condition to RNs and providers
• Support patients and families with education, emotional support, and end-of-life care needs
• Collaborate with interprofessional team to coordinate holistic, patient-centered care
• Follow infection control, safety, and regulatory standards at all times
