LPN Evening

As an Evening LPN at MJHS, you will deliver compassionate, mission-driven nursing care to patients across hospice, home care, and long-term care settings in the greater New York area. You will administer medications, perform treatments, monitor patient status, and provide education and emotional support to patients and families. Working collaboratively with RNs, physicians, and an interprofessional team, you'll help implement individualized care plans that honor dignity and respect.

Responsibilities

• Provide compassionate evening nursing care to patients in hospice, home care, and long-term care settings
• Administer medications and treatments per provider orders and organizational protocols
• Monitor, document, and report patients' condition, vital signs, and responses to care
• Support patients and families with education, emotional support, and care coordination
• Collaborate with RNs, physicians, and interprofessional team members to execute care plans
• Maintain accurate, timely clinical documentation in accordance with regulatory standards
• Follow infection control, safety, and quality assurance procedures at all times
• Participate in ongoing training, huddles, and performance improvement initiatives
