Registered Nurse Wound Care

Job Summary:

Provides specialized wound, ostomy, and continence care to patients, conducting health assessments and developing individualized care plans. Collaborates with the healthcare team to ensure optimal patient outcomes and promotes education and prevention strategies.

Responsibilities:

• Evaluate and document patient responses to wound, ostomy, and continence treatments.
• Develop, implement, and update standards of care, ensuring appropriate supplies, technology, and equipment are utilized.
• Educate patients and families on management, prevention, and self-care techniques related to wounds, ostomies, and continence issues.
• Collaborate with interdisciplinary teams, making referrals and providing recommendations based on assessments.
• Lead staff education initiatives, including orientation, in-services, and ongoing professional development.
• Participate in quality improvement activities and assist in research efforts related to wound care practices.

Qualifications:

• Hold an active New York State RN license verified through NYSED.
• Possess at least 2 years of Med/Surg nursing experience, specializing in wound and colostomy care.
• Certified as a Wound Ostomy Continence Nurse (CWOCN, CWON, CWCN, CWS, or WCC) issued by recognized certifying organizations.
• Maintain current BLS certification through AHA.
• Demonstrate excellent assessment, communication, and collaboration skills.
