Care Rite Centers seeks a Wound Care LPN to provide specialized wound management for post-acute and long-term care residents. In this role, you will assess, stage, and document wounds; perform evidence-based treatments; monitor healing progress; and collaborate with the wound nurse, physicians, therapy, and nursing teams to develop and adjust care plans. You will educate residents, families, and staff on prevention, skin integrity, and pressure injury reduction while maintaining strict infection control and regulatory standards in a fast-paced, resident-centered environment.

Responsibilities

  • Perform comprehensive wound and skin assessments, staging, and documentation for post-acute and long-term care residents.
  • Implement and evaluate evidence-based wound care treatments and dressing changes per provider orders and facility protocols.
  • Monitor healing progress, recognize complications, and promptly report changes in condition to the interdisciplinary team.
  • Educate residents, families, and staff on wound prevention, pressure injury reduction, and skin integrity best practices.
  • Maintain strict infection prevention and control standards in all wound care procedures.
  • Collaborate with physicians, wound specialists, therapy, and nursing staff to develop and adjust individualized care plans.
  • Accurately document care, assessments, and outcomes in the EMR in accordance with regulatory and facility requirements.
  • Participate in quality improvement initiatives related to wound care outcomes and skin integrity.
  • Assist with staff training and competency in basic wound and skin care practices.
  • Support overall unit nursing care as an LPN, including basic clinical tasks within scope of practice.
