Advanced Practice Clinician

Responsibilities

• Conduct comprehensive assessments and develop patient-centered care plans in home and community settings
• Provide advanced clinical care, including evaluation, diagnosis, and management of acute and chronic conditions
• Collaborate with physicians, nurses, therapists, and social workers to coordinate interdisciplinary services
• Educate patients and families on treatment plans, medications, and self-management strategies
• Document care in the EMR, ensuring accuracy, compliance, and timely communication with the care team
• Participate in quality improvement initiatives and support evidence-based practices
• Serve as a clinical resource and mentor to team members, fostering a culture of collaboration and integrity
