Job Summary

The Case Planning Supervisor oversees the day-to-day case management activities for the assigned NSP and LSP programs, including Aftercare. The role ensures the assessment, planning, family engagement, and evaluation process for case planners complies with guidelines, quality assurance standards, and performance indicators for Close to Home programs. Responsibilities include guiding referrals and discharge activities, coordinating and facilitating treatment teams, monitoring referrals and discharge activities, and coordinating linkages with community-based support programs and services.

The position oversees aftercare case planners and ensures all ACS, OCFS, and Court-mandated time frames are met for youth in the programs. The incumbent provides leadership in supervision, case reviews, progress note reviews, and advocacy to meet program goals, quality services, compliance, and best practices. The Case Planning Supervisor also assists with intakes for youth entering CTH NSPs and LSPs.

Job Responsibilities
  • Provide effective ongoing supervision feedback to Case Planners who report to Case Planner Supervisor.
  • Maintain documentation of supervision through supervisory logs and Connections Supervisory Notes for NSP and LSP cases.
  • Regularly review progress notes, court documentation, and other relevant records to ensure compliance and best practices for NSP and LSP youth.
  • Develop and maintain a case planning team in each NSP and LSP facility collaborating with milieu staff.
  • Set high expectations and enforce deadlines to ensure excellence.
  • Monitor Treatment Team meetings including challenges, goals, YLS assessments, treatment plan tool, and PAMS compliance.
  • Attend transition meetings, Service Plan Review meetings, staff meetings, admin meetings, treatment team meetings, red flag meetings, and discharge meetings.
  • Support principles of each facility s Evidence Based Model to help youth become self-sufficient through education and work experience.
  • Collaborate with agency departments (HR, Training, QI) on performance management, interviewing, mandated training, professional development, PAMS Scorecard, Evolve needs, and QI goals.
  • Oversee, monitor, and approve FASP reports in Connections, Transition plans, and all Federal mandates.
  • Ensure records are accurate and complete in compliance with city, state, and agency regulations.
  • Collaborate with the Director of Community Based Programs to submit referrals to aftercare for continuity of services for youth transitioning back to the community.
  • Complete intakes for youth entering and transferring to NSPs and South Ozone LSP.
  • Cover for Divisional Director when needed and cover workload in the absence of a case planner.
  • Perform other related tasks as assigned.
Qualifications
  • Bachelor’s degree required; master’s degree in human services or related field / LMSW preferred.
  • Minimum of two (2) years of experience, with at least one (1) year in a supervisory capacity in child welfare, or a minimum of three (3) years with at least one (1) year in juvenile justice under qualified supervision.
  • Experience with casework documentation in CNNX.
  • Excellent writing and organizational skills.
  • Experience with juvenile justice system–involved youth preferred.
  • Proficient in Microsoft Office suite.
  • Commitment to progressive social justice and anti-racist practices.
  • Belief in an individual’s capacity to change.
  • Bi-lingual (Spanish) a plus.
Supervisory Responsibility

Social work supervisor will supervise 2 Case Planners from Ozone Queens Male Limited Secure Facility and 1 Case Planner from Bensonhurst Brooklyn Male Non-Secure Placement.
