Job Summary:
• This full-time Masters Case Planner position is based in Brooklyn, NY, and involves working with a team of healthcare professionals in the foster care program.
• The role focuses on providing casework services and fieldwork for children with special medical needs and their families, aiming for timely permanency.
• The planner will collaborate with medical staff and community resources to support children and families.
Requirements
• Provide intensive casework services for children and their families.
• Promote the organization's mission and values.
• Collaborate with nurses and social workers in the foster care program.
• Conduct outreach to connect families with community resources.
• Maintain accurate and detailed written case records.
• Conduct home visits in partnership with assigned RN.
• Perform other duties as assigned.
Qualifications
• Bachelor's Degree in Social Work or related field required.
• Foster care experience preferred.
• Bilingual (Spanish/English) preferred.
• Excellent verbal, writing, and organizational skills.
• Flexibility and ability to work under pressure and meet deadlines.
• Computer literacy.
