Case Coordinator

CASES is seeking a Case Coordinator to join their Nathaniel Community Success (NCS) team, an alternative-to-incarceration program serving individuals with mental illness and substance use problems in Manhattan and Brooklyn. The successful candidate will work with a multidisciplinary team to provide individualized case management, link participants to essential services, and offer court-ordered supervision. The goal of the program is to help participants achieve and maintain recovery while reducing criminal recidivism.

The ideal candidate is a passionate individual committed to social justice, evidence-based practices, and recovery-oriented services who wants to make a positive impact in the lives of individuals impacted by the criminal legal system.

Hours:

This position is full-time Monday
- Friday from 9:00 AM - 5:00 PM

Location Address: 25 Chapel Street, Brooklyn, NY 11201. Our office is easily accessible by public transportation.

Workplace Flexibility:
Flexible (for roles that have days that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning).

Salary:
The salary for this role is $56,000 per year

What You Will Be Doing:

  • Collaborate with NCS colleagues to collectively manage the team caseload
  • Provide in-person services in the program office, as well as in the community, with the location of service delivery determined by participant need
  • Conduct on-site program groups according to training and competence to engage participants in on-site services
  • Provide intensive case management services by communicating with community treatment providers, conducting site visits, and attending case conferences to engage and support participants in community treatment
  • Assess participants' immediate needs, legal circumstances, and engagement needs
  • Use Motivational Interviewing to identify risk factors for criminal recidivism, inform participant of risk factors, and identify community resources to support participant to reduce dynamic risk factors and address needs
  • Act as advocate and liaison for participants in securing access to community services, housing, income support, and natural supports such as self-help
  • Complete charting using database following program standards and timelines
  • Involve family, significant others, and participant-identified collateral contacts in service provision
  • Collaborate with team members to provide coverage for after-hours reporting schedule to support participants engaged in employment, vocational training, and education services
  • Provide culturally competent services in accordance with CASES policies and practice
  • Develop and maintain strategic alliances with community providers
  • Any other duties as required by the Team Leader, Director, and Senior Director.

What We Are Looking For:

  • Minimum of two years of experience working with individuals with mental illness, substance use disorders, co-occurring mental health and substance use disorders and involvement in the criminal justice system
  • At least one year of experience conducting group sessions using evidence-based practice
  • Experience understanding various models and theories pertaining to treatment of mental illness and substance use
  • Willingness to learn and adopt evidence-based principles to reduce recidivism risks
  • Extensive knowledge of community treatment, support services, and resources.
  • Highly organized and self-motivated

Preferred

  • Fluency in Spanish
