Social Worker, Family Advocacy & Support Services

Licensed Social Worker (LMSW or LMHC or LCSW Required)

This position is for a Licensed Social Worker (LMSW or LMHC or LCSW required) in Brooklyn, NY, working onsite on a contract basis.

The role involves providing support to children (Age group 3 to 5 years) and families dealing with illness and disabilities, requiring strong communication skills and the ability to work independently and as part of a team. The Social Worker will advocate for students and families, assist with accessing community resources, and collaborate with multidisciplinary teams to ensure comprehensive care and support.

Responsibilities:

• Conduct intakes and maintain case notes, social histories, and related documentation.
• Provide emotional, behavioral, and therapeutic support to students.
• Assist families with accessing resources including housing, Medicaid, SNAP, respite, and other community services.
• Collaborate with families, school staff, and multidisciplinary teams on IEPs, annual reviews, and student transitions.
• Maintain confidentiality and accurate case records.
• Participate in team meetings, trainings, and other school activities as needed.

Requirements:

• NYS LMSW, LCSW, or LMHC required.
• Master's degree in Social Work from an accredited program required.
• 1+ year of social work experience required.
• Experience working with children age group - 3 to 5 years required
• Strong communication, organization, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Ability to read, write, speak, and understand English
• Effective oral and written communication skills
• Organizational skills and ability to meet deadlines
• Proficiency in using personal computers and relevant software
• Maintain strict confidentiality
• Ability to work flexible hours
• Bilingual with Spanish skills are a plus.
• Clinical experience preferred
