Salary Range: $60,000.00 – $60,000.00 per year

The African American Planning Commission, Inc. • Participate on the intake screening assessment team to determine whether a tenant is suitable for independent living.
• Provide tenants with individualized mental health counseling, and co‑lead specific resident population group sessions.
• Use a Person‑Centered approach to help tenants set goals, identify interests, and address concerns.
• Get to know tenants and their psychosocial history to provide strategic interventions and services.
• Provide education/information to adults and families, and modify service plans as necessary in response to tenant progress.
• Participate in appropriate educational activities and department meetings.
• Direct adult tenants and/or families to appropriate community agencies and resources for financial assistance, counseling, rehabilitation, auxiliary care, or help in locating medical supplies, adaptive devices, and more.
• Prepare, write, and maintain all required progress notes, reports, and group notes.
• Maintain a strengths‑based perspective to engage all members of the family to help improve family functioning.
• Conduct visits in the tenant’s unit to ensure compliance with lease and program rules.
• Develop and maintain contacts with various community agencies to gather information on available resources, relay appropriate tenant information, and improve services provided.
• Help tenants understand and proactively address diagnoses (medical, psychiatric, substance abuse, etc.).
• Advocate and connect tenants to medical, mental health, and other supportive counseling services.
• Provide outreach to engage tenants who do not follow through with appointments and case management.
• Link tenants with vital community resources including medical, mental health, substance abuse treatment, and support groups.
• Engage with community partners to provide services onsite and offsite.
• Conduct unit inspections in the tenant’s unit to ensure compliance with lease and program rules.
• Attend all internal trainings, development conferences, and other mandated trainings scheduled by funding agencies.
• Effectively guide tenants through the application, qualification, and re‑determination processes for SSI, SSDI, Medicaid, Medicare, HRA, HPD, SNAP, and other services as needed.
• Manage an assigned caseload of tenants, ensuring comprehensive case management services are provided to both families and single adults.
• Perform other related duties as may be requested by immediate supervisor and, in their absence, the Program Director.

• Bachelor’s degree in a related field (social work, mental health counseling, psychology, sociology, etc.) with at least 2–3 years’ experience in a residential setting.

• Experience with one or more populations including mental health, substance use, inpatient/outpatient treatment, homelessness, and supportive housing.
• Strong verbal and interpersonal communication skills with a focus on providing excellent tenant services.
• Demonstrated ability to interact effectively and collaboratively with a diverse community of residents, program staff, and external partners.
• Ability to exercise good judgment and apply problem‑solving skills.
• Experience working collaboratively in a team‑oriented and outcome‑focused environment.
• Ability to monitor and manage a variety of tasks assigned.

Base Salary: $60,000.00 per year
