Position: RN, Hospice Admission - (Hernando, Pasco, Citrus Counties)

Sign‑on Bonus. Full time, 10am – 9pm, rotating weekends, in Citrus, Hernando and Pasco counties.

Role Overview

The Admission RN assesses patients in their place of residency or in hospitals and initiates care into the services of Chapters Health System. The RN ensures a seamless transition to Care Navigation and serves as an ambassador of the organization.

Qualifications
  • Current RN license in the state where the employee will be working
  • Minimum of one (1) year of nursing experience; hospice or hospital experience preferred
  • Previous experience working with an EMR/EHR system
  • Strong clinical assessment skills; preferred background in emergency department or ICU
  • Ability to effectively communicate in English (verbal and written); bilingual in Spanish preferred
  • Mobile driver: valid driver’s license and automobile insurance per company policy, reliable transportation to meet schedule
  • Active BLS certification for healthcare professionals from the American Heart Association or Red Cross

Job Responsibilities
  • Complete clinical admissions for all company services
  • Explain services to potential patients/families and address questions, while putting the patient/family at ease
  • Identify admission objections, concerns or other entry barriers and provide responses
  • Educate patient/family on admission requirements and address immediate needs
  • Complete informed consents and initial assessments according to regulations and company policy
  • Provide information to physicians and initiate plan of care to address patient’s immediate needs
  • Initiate skilled nursing interventions to prevent complications, alleviate symptoms and maximize physical and emotional comfort
  • Obtain physician orders and complete documentation per company policy
  • Act as company representative at assigned facilities, facilitate referrals to all service lines, and work closely with referring hospitals, physicians, facilities, patients, families, and the general public
  • Communicate frequently with Care Navigation staff to ensure timely follow‑up
  • When assignment is complete, inform the Care Navigation team and prepare for next assignment
  • Provide all necessary clinical communication timely using SBAR
  • Discuss potential needs with after‑hours staff
  • Obtain consents for services and arrange safe discharge efficiently, meeting productivity expectations
  • Obtain medical record information for the referral
  • Develop strong relationships with case managers, physicians, etc. at facilities
  • Work with Professional Relations Representatives to provide education on all available services
  • Provide care for any GIP patients in a hospital, including daily visits, professional management, documentation and efficient discharge
  • During emergencies (e.g., hurricanes), the triage RN may be required to report to a location designated by the company to ensure continuity of services, which may include working ahead of scheduled times or staying overnight(s)
  • Perform other duties as assigned

Compensation
  • Pay Range: $65,996.22 – $98,994.33
  • This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, and ongoing compliance with the Drug‑Free Workplace Policy
