Role Overview
Admission RN assesses patients in their place of residency or in hospitals and initiates care within the Chapters Health System, ensuring a seamless transition to Care Navigation and serving as an ambassador of the organization.
Responsibilities- Complete clinical admissions for all services.
- Explain services to patients/families, answer questions, and address concerns.
- Identify admission objections and barriers, provide appropriate responses.
- Educate patients/families on admission requirements and immediate needs.
- Complete informed consents and initial assessments per regulations and company policy.
- Provide information and initiate plans of care to address patients’ immediate needs.
- Initiate skilled nursing interventions to prevent complications and maximize comfort.
- Obtain physician orders and complete documentation per company policy.
- Act as company representative at assigned facilities, facilitate referrals and communicate with Care Navigation staff.
- Use SBAR for timely communication and discuss potential needs with after‑hours staff.
- Obtain consents for services and arrange safe discharge, meeting productivity expectations.
- Develop relationships with case managers and physicians, work with Professional Relations Representatives.
- Provide care for GIP patients in hospital, manage documentation and discharge.
- Report to designated locations during emergencies and notify authorities per schedule.
- Perform other duties as assigned.
- Current RN license in the state of employment.
- Minimum 1 year nursing experience; hospice or hospital experience preferred.
- Experience with EMR/EHR systems.
- Strong clinical assessment skills, preferably from emergency department or ICU.
- Strong communication skills in English; bilingual Spanish preferred.
- Able to present services professionally and articulately.
- Valid driver’s license and automobile insurance, reliable transportation.
- Team‑oriented, able to manage multiple priorities, handle emotional stress.
- Ability to perform physical requirements: lifting patients, bending, pushing/pulling, kneeling, or with accommodation.
- Strong goal/outcome orientation, time management, customer service focus.
- Active BLS for healthcare professionals from AHA or Red Cross.
Pay Range: $67,976.53 - $. Position requires drug and/or alcohol testing after conditional offer and ongoing compliance with the Drug‑Free Workplace Policy.
