Position: Nurse Practitioner (PRN) - In-Home Health Assessments

Responsibilities
  • Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case‑management referrals, and follow‑ups, documenting all care in accordance with Matrix guidelines and protocols.
  • Administer and order point‑of‑care and lab‑based testing as needed.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Work with Primary Care Physicians to coordinate patient education and follow‑up.
  • Provide services at members’ homes or Skilled Nursing Facilities.

  • Competitive Pay – Earn income per completed assessment: $90–$115. Monthly earnings range from $1,800 to $11,500 for 20–100 visits, varying by state rate.
  • Other Benefits – Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
  • Education and Support – Comprehensive onboarding training, a preceptor, 1:1 feedback, and free continuing education access.
  • Additional Licensure – Company support for obtaining licensure and credentialing in neighboring states when needed.
  • Full‑time Opportunities – PRN providers can transition to full‑time roles and receive a sign‑on bonus.
Qualifications
  • Master’s degree or equivalent experience and satisfactory completion of NP licensure.
  • Current NP licensure in the state of practice, including prescription authority or eligibility for prescriptive authority.
  • Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties.
  • Current BLS, ACLS, or CPR certification.
  • One year of NP experience preferred; new graduates encouraged to apply.
  • Valid state driver’s license and required automobile insurance coverage.
  • Liability coverage: $100,000 per person / $300,000 per incident (bodies injury) and $50,000 property damage.
  • Deductible: $1,000 or less for collision and comprehensive.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills.
