Responsibilities- Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case‑management referrals, and follow‑ups, documenting all care in accordance with Matrix guidelines and protocols.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Work with Primary Care Physicians to coordinate patient education and follow‑up.
- Provide services at members’ homes or Skilled Nursing Facilities.
- Flexibility – Visits are scheduled based on your availability from Sunday to Saturday, 8 am–7 pm. You can work during the day, evening, or weekend.
- Competitive Pay – Earn income per completed assessment: $90–$115. Monthly earnings range from $1,800 to $11,500 for 20–100 visits, varying by state rate.

- Additional Licensure – Company support for obtaining licensure and credentialing in neighboring states when needed.
- Full‑time Opportunities – PRN providers can transition to full‑time roles and receive a sign‑on bonus.
- Master’s degree or equivalent experience and satisfactory completion of NP licensure.
- Current NP licensure in the state of practice, including prescription authority or eligibility for prescriptive authority.
- Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties.
- Current BLS, ACLS, or CPR certification.
- One year of NP experience preferred; new graduates encouraged to apply.
- Valid state driver’s license and required automobile insurance coverage.
- Liability coverage: $100,000 per person / $300,000 per incident (bodies injury) and $50,000 property damage.
- Deductible: $1,000 or less for collision and comprehensive.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
