Thank you for your interest in joining our team! Social workers provide a wide range of services including emotional support, counseling, assessment of patient and family needs, treatment planning, referral to community resources, education to patients, families and hospital staff regarding health, safety and psychosocial issues, and collaboration with physicians and patient care staff.
Essential Duties and Responsibilities- Maintain utmost level of confidentiality at all times.
- Adhere to hospital policies and procedures and ethical standards of corporate compliance and integrity guidelines.
- Follow The Driscoll Way customer service standards set forth by the Hospital.
- Maintain annual web‑based education requirements.
- Follow the Social Work Code of Conduct set by the Texas State Board of Social Work Examiners.
- Provide professional supervision as required and pursue continuing education to maintain state licensure.
- Attend and participate in staff meetings, training of new employees, supervision of social work students, cross‑training of co‑workers, and provide peer support.
- Participate in monthly and holiday call schedule and report to work when on call.
- Travel to ambulatory clinics for education, patient assessment, and meetings as requested.
- Review and revise community resource lists, annual performance improvement activities, policy and procedures, budget planning.
- Assist with staffing schedule voluntarily or as assigned by supervisor.
- Conduct process and debriefing support opportunities for staff concerning grief, conflict, and interaction with families.
- Collect data for research relating to social work issues.
- Respond to physician orders and nursing functional screening requests in a timely manner.
- Make daily rounds in assigned units and proactively screen for high risk, determine need for and eligibility for services, identify strengths/coping capabilities, and assess informal and formal network of support through psychosocial assessment.
- Provide comprehensive psychosocial assessment, evaluation, treatment planning, education, and discharge planning to address barriers to care such as social support limitations, mental health and substance abuse issues, family violence, emotional stability, and behavioral risks.
- Assist families in understanding medical and treatment information.
- Provide mediation and crisis intervention for families to ensure safety and adherence to medical directives.
- Provide supportive end‑of‑life care including assessment, advocacy, grief and crisis counseling, resource counseling (home care, hospice, financial and legal needs, advance directive planning, funeral assistance), memory packets, and team‑care planning.
- Assist in linking families to services provided by Medicaid Transportation Program for transportation, lodging, and meals.
- Coordinate and participate in discharge planning activities, including care conferences, referral to community resources, follow‑up care, secure needed equipment, supplies, services, and planning for appropriate school services for homebound patients and students with disabilities.
- When abuse or neglect is suspected, notify appropriate agencies and serve as liaison between the hospital and agencies for discharge planning.
- Complete timely documentation in the medical record of contact with patients and families.
- Plan, coordinate, and attend team and family conferences and actively participate as an advocate in the review and planning of patient care.
- Provide ongoing emotional support for long‑term patients and their families.
- Offer professional consultation to physicians and hospital staff regarding psychosocial, bioethical, and discharge planning issues.
- Conduct and lead support groups for patients, parents, and families and serve as liaison and advocate with community programs and agencies related to health and welfare.
- Provide education to school personnel, students, and community as requested.
- Work with staff from outside agencies to ensure quality care along the continuum.
- Testify in court regarding child abuse and/or neglect.
