- Paid Time Off—beginning at 16 hours per month

- Assist individuals in identifying and articulating their needs for services and supports.
- Coordinate, negotiate, and facilitate a comprehensive array of services and supports to address identified goals and desired outcomes.
- Provide behavioral intervention services, training, and support tailored to the needs of the assigned caseload.
- Ensure that planned services and supports are effectively implemented and properly documented.
- Connect individuals with appropriate community resources, services, and supports as needed.
- Maintain accurate and timely documentation of all meetings, progress notes, and activities related to individual service plans.
- Primary responsibilities include screening, intake assessments, crisis intervention, development of service plans and the implementation and monitoring of services for consumers.
- Self‑motivated with the ability to work independently and take initiative.
- Strong organizational and time management skills.
- Excellent verbal and written communication skills in English.
- Ability to collaborate effectively with diverse individuals, interdisciplinary teams, and systems.
- Proficient in accurately and objectively recording data and documentation.
- Ability to follow established service implementation processes to meet the needs of individuals served and adhere to funding source requirements.
- Skilled in emergency and crisis intervention techniques.
- Proficient in the use of computers and electronic health record (EHR) systems.
Education and Experience
- Bachelor’s or Master’s degree in a Human Services‑related field such as:
- Psychology
- Sociology
- Social Work
- Rehabilitation
- Counseling
- Human Growth and Development
- Gerontology
- Special Education
- Educational Psychology
- Early Childhood Education
- Early Childhood Intervention
- Licensed Registered Nurses are also eligible for this position.
- Valid Driver’s License required.
