Registered Nurse – Visiting Nurse / Nurse Case Manager – Brunswick, SC
Job Summary: The Visiting Nurse (Nurse Case Manager) will visit patients at homes, long‑term care facilities (ALF and ILF), and other settings within the designated service area to evaluate patient health, facilitate care, and initiate telemedicine visits. This full‑time position requires reliable transportation, a personal vehicle and a driven commitment to providing quality healthcare in compliance with all applicable laws and regulations.

8‑hour shifts, Monday‑Friday (8:00 am – 5:00 pm). On‑call duties may be required.
Service AreaBrunswick area.
Responsibilities- Facilitate receipt of all medical records from the patient’s primary provider and specialists.
- Review medical records and complete consents with patients.
- Enroll eligible patients in Care Management.
- Initiate a Care Management Plan of Care.
- Capture all diagnoses at the highest specificity and ensure they are accepted.
- Complete AWV’s for provider review.
- Complete cognitive impairment screenings.
- Complete Social Determinants of Health (SDoH) assessments and/or screenings.
- Complete ACP’s and review with the patient and provider.
- Evaluate for home health, hospice, palliative, or consults with Your Health Specialty Division.
- Evaluate for RPM devices and other resources that may improve patient quality of life.
- Communicate and coordinate care with all care team members.
- Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
- Evaluate for adaptive equipment and DME.
- Evaluate for safe environment.
- Evaluate acute conditions or follow‑up from previous visits.
- Document and log Care Management activities accurately and in a timely manner.
- Maintain current Care Management care plans.
- On‑call may be required.
- Registered Nurse preferred; must hold a current nursing license in good standing.
- Minimum of three (3) years of clinical experience preferred.
- Experience in community settings preferred.
- Proven ability to communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
- Strong written and verbal communication skills.
- Basic computer knowledge.
- Leadership and interpersonal skills.
- Ability to foster a cooperative work environment.
- Team player capable of managing multiple responsibilities with sound judgment.
- Must be able to work flexible hours, travel between offices, facilities, etc., and possess a reliable, insured personal vehicle.
