• Provide 1:1 care following the client's plan, maintaining dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as per plan and assessments.
• Accurately document nursing actions, communications with family, case manager, physicians, and progress toward goals.
• Coordinate or order supplies, medications, and feedings to ensure adequate stock.
• Ensure all patient needs meet physician instructions.

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.
