Perinatal Health Nurse

Responsibilities

• Provide home visitation services to medically diagnosed high‑risk pregnant, post‑partum, and inter‑conceptual women.
• Deliver nurse case management, including referrals for support, education, and community resources.
• Conduct outreach to reduce disparities in birth outcomes among target populations.
• Provide general clinical support within the home.
• Communicate client status updates to health care providers throughout the program.
• Maintain a valid driver’s license to travel as required.

• Associate degree in nursing from an accredited college or university.
• Completion of a PHN course within one year of hire.
• Current Georgia Registered Professional Nurse license.
• CPR certification.

• PH Nurse 1:
  Salary $51,811.56
• PH Nurse 2:
  Salary $58,547.06 (requires one year nursing experience or equivalent; PH Registered Nurse 1 or position equivalent).
• PH Nurse 3:
  Salary $66,168.17 (requires one year nursing experience or equivalent; PH Registered Nurse 3 or position equivalent).

• BSN.
• Previous experience in home care and/or medical case management.
• Experience with newborns and/or pediatrics.
• Bilingual in Spanish.
• At least one year of Labor & Delivery experience, or one year working in a prenatal care clinic, or one year experience working with mothers and babies.

All Coastal Health District County Health Department employees are deemed essential employees in the event of an emergency. You may be called on to participate in staffing a shelter, a Specimen Point of Collection Site (SPOC), or a Point of Dispensing site (POD). Participation is a part of your job requirement, whether the emergency occurs during or outside regular work hours.
