Center Well Home Health is seeking an experienced Home Health LPN to provide one-on-one nursing care in patients’ homes. Reporting to the Clinical Manager, you will implement care plans, educate families, and maintain thorough documentation to support recovery and independence in daily living.
The role requires two years of LPN/LVN experience, a current nursing license in the state, CPR certification, and reliable transportation. A 40-hour week with a competitive benefits package is offered.
