The Crisis Coordinator - CCBHC provides crisis intervention services, assessment and stabilization support to individuals and families experiencing a behavioral health crisis within the community.

This role delivers direct, short-term stabilization services and provides follow-up support for up to 30 days of clinically indicated follow-up care. Additionally, the Coordinator serves a bridge, ensuring seamless collaboration, coordination, and communication between Sweetser’s Certified Community Behavioral Health Clinic team and external Mobile Crisis Providers.

ESSENTIAL FUNCTIONS:

• Responds to walk-in requests for crisis intervention, consultation, and support from the person served, family members, and community-based service providers in order to assist in the stabilization of crisis situations involving children and/or adults who are experiencing an acute psychiatric crisis.
• In collaboration with the person served, determines the need for a face-to-face Mobile Crisis Assessment and, if needed, contacts 988/Mobile Crisis Provider to schedule a meeting site that is convenient for the individual and provides safety for all involved. Mobile Crisis staff are encouraged to provide services in the least restrictive setting that supports the comfort and dignity of the person served.
• Communicates directly with persons experiencing a crisis in order to assess mental status, current level of functioning, and the person’s wishes and goals for crisis stabilization. The liaison will collaborate with the Mobile Crisis Provider as needed.
• Develops a crisis stabilization plan with the person served and family or guardian, if involved, based on a comprehensive crisis assessment that includes mental status, level of care determination, and risk assessments.
• Collaborates with the Mobile Crisis Provider when they facilitate voluntary or involuntary hospitalization in accordance with state regulations, and provides updates to the Sweetser CCBHC Clinic Team.
• If Mobile Crisis is not activated, the Liaison will develop and participate in the implementation of crisis follow-up by providing stabilization support with the CCBHC Clinic Team for up to 30 days.
• Assists the CCBHC Clinic Team in updating Safety Plans as needed to address future crisis concerns.
• May facilitate referrals to community programs and the CCBHC for needed services (i.e. case management, in-home supports, outpatient therapy, etc.).
• Ensures that proper releases, consents, and service agreements are acquired at the time of assessment.
• Ensures communication with the treatment team at the time of assessment, collaborating with 988/Mobile Crisis Provider as clinically indicated.
• Collaborates with the individual’s providers and natural supports.
• Meets recordkeeping requirements through the timely completion of assessments and by maintaining current progress notes, logs, and other information as required in the client’s CCBHC chart.
• Actively participates in providing clinical in-service training and teaching to colleagues, community groups, etc., as appropriate and requested by the program supervisor or designee.
• Attends treatment team and service planning meetings when requested.
• Attends clinical supervision and staff meetings as required, or as needed.
• Provides reciprocal communication between the Sweetser Certified Community Behavioral Health Clinic team and the Mobile Crisis Provider.
