For a limited time Maine Health is offering a $10,000 Sign on bonus for all eligible Registered Nurses with 0-2 years of RN experience and $20,000 for Registered Nurses with greater than 2 years of RN work experience!

• Eligible candidates are hired (offer accepted) into a Full or Part time RN position. Bonus amount prorated for Part time hires, per diem hires are ineligible.

• Current Maine Health member employees are ineligible; former Maine Health Members are ineligible until greater than 6 months separation from employment.

Summary:

This is a 40-hour a week, Day shift position with multiple shift-start options.

As a Centralized Post-Acute Bed Placement Coordinator , you'll play a pivotal role in helping patients transition safely and successfully to the next level of care. Working across our health system, you'll partner with care teams, patients, families, and community providers to ensure patients receive the right care, in the right setting, at the right time.

• Coordinate placements for patients transitioning from acute care to post-acute settings, including skilled nursing facilities, inpatient rehabilitation, long-term acute care, home health, hospice, and other community-based services.
• Serve as a clinical resource for determining the most appropriate level of post-acute care based on patient needs, goals, and payer requirements.
• Collaborate closely with hospital teams, post-acute providers, patients, and families to facilitate timely, seamless care transitions.
• Review clinical information and assess care needs to support placement decisions and referral processes.
• Build and maintain strong relationships with post-acute care partners across the region.
• Help improve patient flow, reduce barriers to discharge, and ensure patients receive high-quality care throughout their recovery journey.

• Education:
  
  Associate’s degree in appropriate clinical field required; bachelor’s degree preferred.
• License/
  
  Certifications:
  
  Currentlicensure as a Registered Nurse, Physical Therapist or Occupational Therapist in the State of Maine required. Current Basic Life Support (BLS) certification is required or must be obtained within 30 days of start date. Case Management certification (CCM, ACM-RN, or equivalent) preferred.
• Experience:
  
  Three years’ direct clinical experience required. Experience working in a skilled nursing facility or post-acute entity, discharge planning, utilization management, admission management, or post-acute care coordination preferred. Experience in a healthcare setting involving care transitions across multiple levels of care preferred.
