The Case Manager RN provides leadership in coordinating patient-centered care, developing discharge plans, and collaborating with multidisciplinary healthcare teams to ensure optimal transitions in care.

• Problem-solve and actively pursue resolution.
• Communicate directly with staff, physicians, patients, and families.
• Model leadership behavior through courtesy, respect, and efficiency.
• Coordinate patient care processes to achieve quality outcomes.
• Facilitate patient and family education to promote continuity of care.
• Review patient plans of care with the multidisciplinary team.
• Communicate with patient care staff for continuous assessment and planning.
• Function without direct supervision, organizing assignments for productivity.
• Adhere to name badge and dress code compliance.

• Associate’s degree in Nursing required.

• 3+ years of experience in a hospital, acute care, or direct care setting required.

• Current, unrestricted compact RN license in the state of residence required.
