Overview
Restore Strength. Enhance Mobility. Transform Lives rtland Home Health is looking for a Full‑Time Physical Therapist to join our team in Ft. Wayne, IN. We provide skilled rehabilitation services that empower patients to recover, restore function, and live safely at home.
Licensure:
Physical Therapy – Licensure in IN.
Monday‑Friday, 8 am‑5 pm. Salary, full benefits, 401(k) match, and a $5,000 sign‑on bonus.
What You’ll Do- Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
- Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
- Adjust treatment plans and techniques as needed to achieve optimal outcomes.
- Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
- Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
- Document all visits, assessments, and progress notes accurately and in a timely manner.
- Support patients in maximizing their independence and improving quality of life—wherever they call home.
- Master’s degree in Physical Therapy from an accredited program.
- Current PT license in the state of employment (or eligibility to obtain).
- CPR certification required.
- Strong communication, assessment, and problem‑solving skills.
- Ability to work independently in a variety of home environments.
- Valid driver’s license, auto insurance, and reliable transportation.
- 1+ year of experience in a hospital, home health, or outpatient rehab setting.
- In‑home therapy delivery.
- Geriatric rehab, orthopedic or neurologic rehab.
- Post‑acute care or chronic condition management.
- Experience working as part of a mobile or interdisciplinary care team.
