As a Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare & Hospice
, you will play a key role in an organization with a reputation for unsurpassed quality care, a close-knit, family-friendly culture, and a supportive management team as we begin to introduce hospice services to our Bryan area territory. We will rely on you to assess, plan and implement patient care; educate patients and families on comfort care processes, procedures and medications;

and address various patient needs.

• One year of comfort care experience
• Current RN state licensure
• CPR certification
• Ability to travel for in-home patient visits
• Exceptional time-management skills
• Excellent interpersonal and customer service skills
• Hospice/case management experience preferred
• This position will include an on-call rotation.
