Hospice Registered Nurse

Position

Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare & Hospice.

Bryan, OH (serving Williams, Defiance, Fulton and Henry Counties).

Assess, plan and implement patient care; educate patients and families on comfort care processes, procedures and medications; address diverse patient needs across the community.

• Assess and develop individualized care plans for hospice patients.
• Implement and monitor comfort care treatments and medication.
• Educate patients, families, and caregivers on palliative and end‑of‑life care.
• Coordinate with interdisciplinary team to provide continuity of care.
• Document patient assessments, interventions, and progress in electronic charting.
• Participate in on‑call rotation and travel for in‑home patient visits.

• Registered Nurse (RN) license in Ohio.
• Minimum one year of comfort care experience.
• CPR certification.
• Excellent time‑management, interpersonal, and customer‑service skills.
• Ability to travel for in‑home patient visits.
