As a Service and Support Administrator (SSA), you'll be the central connection for individuals and families receiving our services. You'll combine the meaningful work of direct advocacy with strategic coordination, helping people of all ages identify their strengths, build their support networks, and pursue their dreams. While your caseload will be primarily youth and young adults, you'll have the opportunity to work across the full spectrum of SSA services and understand how individuals transition through our programs.
You'll help shape person-centered plans, coordinate community resources, and be a trusted guide through the developmental disabilities services landscape. You'll join a compassionate, mission-driven team dedicated to making real, lasting differences in people's lives.
Service Coordination & Support:
- Serve as the primary point of coordination for individuals and families receiving services
- Assess individual choice, including their passions, interests, dreams, and any known or likely risks
- Identify supports that promote rights, self-determination, physical and material well-being, personal development, interpersonal relationships, and social inclusion
Person-Centered Planning & Assessment:
- Develop, review, and revise Individual Service Plans (ISPs) using person-centered planning principles
- Establish recommendations for and obtain approval of service budgets based on assessed needs
- Facilitate communication and coordination among individuals, families, and team members to ensure everyone has current ISP information
- Implement continuous review processes to ensure services and supports align with the plan
- Document case management notes in Britco software for Targeted Case Management reimbursement
Community Collaboration & Resource Development:
- Collaborate with schools, community agencies, and service providers to coordinate comprehensive services
- Connect individuals and families to community resources that meet identified needs
- Assist with OOD referrals and coordinate employment services for individuals seeking community employment
- Maintain comprehensive records according to federal, state, and county board standards

- Carry on-call phone in rotation
- Respond to events to ensure individuals' health, safety, and wellbeing
