Vantage Hospice is expanding into the Bryan/College Station area! We are growing and seeking an experienced, compassionate RN Case Manager to join our team.
Position
Reports To:
Clinical Manager
Job Type: Full-Time
Qualifications- Current licensure/registration as a Registered Nurse (RN) in the State of Texas.
- Previous hospice nursing experience preferred.
- Experience caring for terminally ill patients and families with strong knowledge of symptom control and emotional support.
- Background in Oncology, Home Health, or Long-Term Care is a plus.
- Excellent physical and psychosocial assessment skills with strong reasoning, deduction, and critical thinking abilities. Must demonstrate independence in clinical assessment.
- Ability to prioritize urgent patient, family, and facility needs while managing multiple tasks simultaneously.
- Strong time-management and organizational skills with flexibility in scheduling.
- Must display a professional attitude and demeanor at all times.
- Reliable transportation, valid Texas driver’s license, and state-required liability insurance (or access to reliable transportation) required.

- Conduct comprehensive patient assessments and develop individualized hospice care plans.
- Coordinate and manage patient care services while ensuring compliance with HIPAA and all regulatory guidelines.
- Collaborate with the interdisciplinary team to support discharge planning and post-acute care needs.
- Educate and support patients and families regarding hospice services and end-of-life care.
- Maintain accurate, timely documentation of patient progress and care interventions in accordance with agency policies.
