Position Overview

We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to provide high-quality, patient-centered care to hospice patients in their homes throughout Pulaski, Saline, Jefferson, and surrounding counties. This role involves case management, patient assessments, care planning, interdisciplinary collaboration, and supporting patients and families through every stage of hospice care. The ideal candidate will have hospice experience, strong communication skills, and proficiency with Homecare Homebase (HCHB).

Schedule

& Work Details
  • Coverage Area: Pulaski, Saline, Jefferson, and surrounding counties
  • Schedule: 8:00 AM - 4:30 PM
  • Weekly Hour Guarantee: 32 hours
  • Pay Rate: $51.00 - $53.00 per hour
  • On-Call/Weekend Requirement: Negotiable
  • Mileage Reimbursement: Yes, reimbursed between patient visits
Key Responsibilities
  • Provide skilled nursing care to hospice patients in their home settings.
  • Conduct comprehensive patient assessments and monitor changes in condition.
  • Manage an assigned caseload and coordinate all aspects of patient care.
  • Develop, implement, and revise individualized plans of care in collaboration with physicians and the interdisciplinary team.
  • Participate in Interdisciplinary Group (IDG) meetings and contribute to care planning discussions.
  • Educate patients and families regarding disease progression, symptom management, medications, and end-of-life care.
  • Complete timely and accurate clinical documentation in accordance with agency, state, federal, and hospice regulatory requirements.
  • Coordinate services with physicians, social workers, chaplains, aides, and other members of the care team.
  • Perform hospice admissions and needs assessments as appropriate.
  • Travel daily between patient residences within the assigned coverage area.
  • Ensure high standards of patient satisfaction through compassionate communication and exceptional bedside manner.
Required Qualifications
  • Active Registered Nurse (RN) license.
  • Minimum Associate Degree in Nursing (ADN).
  • Hospice nursing experience required.
  • Proficiency with Homecare Homebase (HCHB).
  • Strong communication, interpersonal, and patient education skills.
  • Excellent bedside manner and ability to build rapport with patients and families.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to work independently and effectively manage a field-based caseload.
Preferred Qualifications
  • Experience conducting hospice needs assessments and admissions.
  • Previous case management experience in a hospice setting.
  • Strong organizational and time management skills.
  • Ability to collaborate effectively within an interdisciplinary care team.
  • Experience serving diverse patient populations in home-based care environments.
