* We're seeking an experienced  
** Travel Registered Nurse (RN)
** with Home Health and Hospice experience to join a compassionate hospice team serving patients throughout the Bryant, Arkansas area. This is an excellent opportunity for an RN who enjoys providing patient-centered care in the home while managing an independent caseload.

** Schedule*
* + Monday-Friday

+ 8:00 AM - 4:30 PM

+ Weekly Hour Guarantee: 32 hours

+ Productivity Expectation: 24 points per week

+ On-call/weekend requirement:
Negotiable

** Coverage Area*
* + Pulaski County

+ Saline County

+ Jefferson County

+ Surrounding Arkansas counties

** Responsibilities*
* + Provide compassionate hospice nursing care to patients in their homes.

+ Perform comprehensive patient assessments and develop individualized care plans.

+ Manage an assigned hospice caseload while coordinating care with the interdisciplinary team.

+ Participate in Interdisciplinary Group (IDG) meetings to review patient progress and update care plans.

+ Complete hospice admissions and ongoing needs assessments.

+ Educate patients and caregivers on disease progression, medications, symptom management, and end-of-life care.

+ Maintain accurate documentation within the electronic medical record.

+ Travel daily between patient homes while delivering exceptional, patient-centered care.

** Requirements*
* + Active Registered Nurse (RN) license

+ Minimum Associate Degree in Nursing (ADN)

+ Home Health and Hospice experience

+ Experience using Homecare Homebase (HCHB)

+ Excellent communication, assessment, and bedside manner

+ Ability to work independently while managing a patient caseload

** Preferred Qualifications*
* + Experience performing hospice admissions and comprehensive hospice needs assessments

+ Strong case management skills

** Additional Information*
* + Mileage reimbursement provided between the first patient visit and the last patient visit of the day.

+ Flexible on-call schedule based on team needs.
