Gentiva Hospice in Bryant, AR is seeking a Licensed Practical Nurse (LPN) Patient Care Coordinator to manage day-to-day hospice operations. You will schedule visits, coordinate with RNs, social workers, and chaplains, and support IDT meetings to ensure timely, compassionate patient care under RN supervision.
Requires AR LPN license, CPR, 3–5 years in hospice/home health, driver’s license, and familiarity with EMR and MS Office for records, benefits, and transitions.
