The Licensed Practical Nurse is responsible for professional nursing care to clients. This role works closely with Registered Nurse, clinicians, administrative staff, and insurance companies to support the financial and operational health of the organization.
Essential Job Functions- Assist with pre-certification processes for insurance clients, as directed.
- Perform and document initial and ongoing nursing observations of each patient’s physical and mental status in accordance with program protocols, using interview techniques and basic assessment skills; promptly report findings to the RN or physician.
- Accurately document care provided and assist in preparing required reports and/or data as requested.
- Follow established emergency screening protocols and promptly notify and consult with RN, physician, or on‑call staff per procedures.
- Contribute to the assessment of client needs and assist in implementing care plans under the direction of an RN or physician.
- Collect specimens and perform basic clinical tests as authorized; record and report results to the RN or physician (does not independently interpret results).
- Assist in completing nursing assessments, including gathering data for Mental Status Exams on new admissions, for review by the RN.
- Utilize approved crisis intervention techniques to support de‑escalation and ensure client safety; report incidents to supervising staff.
- Function as a collaborative team member, supporting implementation of health promotion and behavioral health interventions under supervision.
- Provide basic health education and reinforce teaching plans developed by the RN or physician in individual and group settings.
- Work under the supervision of an RN and/or physician to implement established plans of care for clients.
- Participate in required trainings and Performance Improvement activities.
- Maintain confidentiality of client records in accordance with HIPAA standards.
- Knowledge of and experience using behavior modification techniques.
- Flexibility to effectively deal with competing demands.
- Ability to organize and achieve quality performance level and adapt to new routines, procedures, and skills.
- Ability to demonstrate a high level of overall nursing skills.
- Proficient in coordinating patient treatment activities to allow for optimal patient care.
- Excellent verbal and written communication skills, with the ability to interact effectively with patients, payers, and team members.
- Strong problem‑solving abilities with a focus on resolving billing issues and ensuring accurate claim submission.
- Ability to manage multiple tasks, prioritize workload, and meet deadlines in a fast‑paced environment.
- Licensed Practical Nurse in the State of West Virginia.
- Valid driver’s license and the ability to obtain and maintain West Care’s vehicle insurance.
- Minimum of 2 years of experience in nursing or a related field.
- Environment:
Office setting, with regular use of computers and telephones. - Hours:
Typical work hours are Monday to Friday, with occasional after‑hours work as needed.
- Requires mobility and physical activity: adequate range of body motion and mobility to work in an office, residential, or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended periods of time, bending, twisting, reaching, balancing, and occasional lifting and carrying of up to 50 pounds. Use of computer and telephone systems is required which includes coordination of eye and hand, and fine manipulation by the hands (typing, writing, and working with files).
