LPN​/LVN

Join Sevita as an LPN/LVN and provide compassionate, person-centered care in community and home-based settings. In this role, you'll deliver direct nursing care, administer medications, monitor health status, and support individuals with behavioral health, brain injury, and complex medical needs. Collaborate with an interdisciplinary team to implement care plans, educate individuals and families, and document services accurately. Responsibilities

• Provide direct nursing care to individuals in home and community-based settings
• Administer medications and treatments per physician orders and state scope of practice
• Monitor, document, and report changes in health status using EMR systems
• Implement individualized care plans in collaboration with RNs and interdisciplinary teams
• Support individuals with behavioral health, brain injury, and complex medical needs
• Educate individuals and families on health conditions, medications, and self-care
• Ensure compliance with infection control, safety, and regulatory standards
• Coordinate care and communicate effectively with physicians and team members
• Participate in ongoing training, competencies, and professional development
• Contribute to a supportive, inclusive culture focused on person-centered care
