We are seeking a dynamic and compassionate Family Nurse Practitioner FNP to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary care services, perform advanced clinical assessments, and develop personalized treatment plans for a diverse patient population across all ages. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions in an outpatient clinic.

Responsibilities:

• Conduct Thorough patient assessments, including vital signs, physical examinations, and medical History reviews, to determine health status.
• Diagnose and treat acute illnesses and chronic conditions using evidence-based practices.
• Prescribe Medications in accordance with state regulations.
• Manage electronic records EHR/EHR system and ensure seamless medical documentation for quality assurance

Qualifications:

• Valid state licensure as a Family Nurse Practitioner with current certification from the American Association of Nurse Practitioners
• Must be bilingual (English/Spanish)
• Demonstrated experience working in primary care settings with proficiency in a medical office
• Strong clinical skills including physical examinations ,diagnostic evaluation.
• Experience working with diverse populations, including pediatrics, geriatrics, and individuals with disabilities.
• Familiarity with EMR/EHR systems.
