Patient Care Coordinator

Responsibilities

• Develop and maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company services available in the market.
• Conduct facility visits at physician requests to assist program clinical team in determining eligibility.
• Ensure effective communication and collaboration with program staff and other field sales resources via weekly meetings; actively participate in weekly program business development meetings.
• Assist program in processing physician orders as directed.
• Report to a Market Executive of Sales.
• Patient‑facing role as part of our TB screening program; if selected, required to be screened for TB.

• RN/LPN/LVN license required.
• Bachelor's of Science in Nursing preferred.
• 3 years clinical experience preferred.
• Knowledge of governmental regulations, Medicare eligibility requirements and understanding of potential care plan needs.
• Excellent customer service, account development, organization, time management, problem‑solving, communication and selling skills.
• Will travel within an assigned territory and to sales meetings as required.

$65,200 - $89,000 per year
