Position: RN Director of Health Services DHS 10000 Bonus

Director of Health Services (RN) – Buffalo, MN

Park View Assisted Living and Lake Ridge Assisted Living are hiring a Director of Health Services (RN) to lead the clinical team and ensure quality care across two campuses in Buffalo, MN.

Position Type: Full-Time. Will float between both sites.
Wage Range: $90,000 to $100,000 annually, depending on experience.
Bonus: $10,000.
Locations: 613 Montrose Blvd, Buffalo, MN 55313 (Park View); 310 Lake Blvd S, Buffalo, MN 55313 (Lake Ridge).

Responsibilities
  • Complete initial assessment of potential Independent, Assisted, and Memory Care residents.
  • Create and implement individual care plans for each resident.
  • Ensure residents’ rights are protected and advocate on their behalf.
  • Communicate with healthcare providers, county personnel, and families regarding residents’ needs and health status.
  • Take fiscal responsibility for setting and staying within budgets for nursing and care-related areas.
  • Monitor and maintain resident charts and progress notes.
  • Oversee ordering, setting up, and administration of all medications.
  • Fulfill staffing requirements for the nursing department, including hiring, evaluation, and scheduling.
  • Provide health and medication training to nursing personnel and document ongoing education to ensure regulatory compliance.
  • Supervise nursing staff, offering guidance, coaching, and discipline to maintain a high standard of care and a positive work environment.
Qualifications
  • Current Minnesota RN license.
  • Experience supervising staff preferred.
  • Previous experience in an Assisted Living setting.
  • Familiarity with Assisted Living rules and regulations.
  • Strong leadership skills.
  • Effective communication with residents, families, and staff, fostering a positive nursing department atmosphere.
