As a RN/LPN Visit Nurse – Hospice, you will provide skilled nursing care to hospice patients in their homes while collaborating with an interdisciplinary team to ensure comfort, dignity, and quality of life. Our ideal candidate is compassionate, organized, and committed to delivering exceptional care while building meaningful relationships with patients and their families.

Full-Time, benefit-eligible position based on our West Metro area.

$32 - $44 /hour depending on licensure and experience

• Provide skilled nursing care to hospice patients according to individualized care plans and physician orders.
• Assess patients' health status, monitor vital signs, and report changes in condition to the RN Case Manager.
• Collaborate with physicians, families, and the interdisciplinary care team to coordinate quality patient care.
• Obtain and implement physician orders and reorder medications as needed.
• Complete accurate and timely electronic documentation, with the majority of documentation completed at the point of care.
• Schedule and complete patient visits during regular business hours and participate in weekend/holiday rotation as assigned.
• Participate in case conferences, interdisciplinary team meetings, staff meetings, and ongoing educational opportunities.
• Maintain compliance with all state and federal home health and hospice regulations.
• Perform other duties as assigned.

• Current RN or LPN license in the State of Minnesota.
• Minimum of 2 years of experience in geriatrics, home care, hospice, or a related healthcare setting.
• Strong knowledge of nursing practices and principles.
• Excellent assessment, communication, and interpersonal skills.
• Ability to establish positive relationships with patients, families, physicians, and interdisciplinary team members.
• Strong organizational and documentation skills with proficiency using electronic medical records.
• Ability to exercise sound clinical judgment while maintaining compassion and professionalism.
• Knowledge of HIPAA guidelines and commitment to maintaining patient confidentiality.
