Job Description
On-Call Triage Nurse (RN)
Pay: $40/hour + On-Call Stipend
We are seeking an experienced Registered Nurse (RN) to join our team as an On-Call Triage Nurse. This position provides after-hours clinical support to Assisted Living communities by triaging resident concerns, guiding caregiving staff, and coordinating care to ensure resident safety and quality outcomes.
Responsibilities
- Receive and triage calls from Assisted Living staff, including CNAs, CMAs, Medication Technicians, and other caregivers.
- Assess resident needs using nursing judgment and provide appropriate clinical direction.
- Communicate significant changes in resident condition to physicians, nursing leadership, and responsible parties.
- Support care coordination and individualized resident care plans.
- Respond calmly and professionally to urgent and emergency situations.
- Document assessments, incidents, interventions, and communications accurately.
- Review and process physician orders and medication changes as needed.
Qualifications
- Active Registered Nurse (RN) license in the state of practice.
- Graduate of an accredited nursing program.
- Minimum of 2 years of nursing experience required.
- Assisted Living, long-term care, or dementia care experience preferred.
- Strong assessment, critical thinking, communication, and documentation skills.
- Ability to work independently and make sound clinical decisions.
- Must successfully complete a background check and drug screening.
