Role OverviewWe are seeking an organized, empathetic, and proactive HCSS Liaison to join our Waiver Department. You will be the critical link between clients, caregivers, service coordinators, and Buffalo Homecare’s administrative teams—ensuring smooth coordination of Home and Community Support Services (HCSS) for waiver participants. Pay rate will be determined based on experience.
Key Responsibilities- Serve as the main point of contact for waiver clients, families, caregivers, and internal staff.
- Coordinate HCSS aide schedules and assignments to ensure consistent, high-quality care.
- Post and search for PCA cases to quickly fill open shifts and meet client needs.
- Ensure compliance with all applicable waiver program guidelines and internal policies.
- Accurately document updates, client interactions, and service changes in agency systems.
- Proactively resolve issues, concerns, or scheduling conflicts.
- Collaborate across the Waiver Department, clinical teams, and administrative staff to uphold service excellence.
- Prior experience in waiver program support, home care coordination, or related human services roles is strongly preferred.
- Excellent ability to communicate clearly and handle multiple priorities.
- Familiarity with Medicaid Waiver Programs such as NHTD or TBI is a plus.
- Comfort with scheduling and documentation software.
- Must demonstrate professionalism, reliability, and a driven work ethic.
