The Clinical Pharmacist will participate in day-to-day clinical pharmacy operations through collaborative practice agreements and will collaborate with primary care providers, clinic team members and pharmacy team members to provide a cohesive care team to improve the health of patients.
As part of the essential functions of this role, the Clinical Pharmacist:
- Provides and oversees chronic disease state management utilizing collaborative drug therapy management (CDTM) for clinic patients
- Assesses patients on referral, screens for drug interactions and adverse effects, provides disease/medication education, and makes recommendations to optimize and/or simplify medication regimens
- Inputs and/or verifies all patient demographics, including allergy information, OTC and herbal usage
- Complies with all OBRA regulations for patient counseling including but not limited to administration instructions, side effects, proper storage and potential drug-drug and drug-nutrient interactions
- Participates in committees including Population Health Operation Committee, Diabetes Management Committee, HIV Clinical Quality Committee, Medical Providers Committee
- Attends all clinic-wide meetings
Qualified candidate
will have a Doctor of Pharmacy and completion of PGY-1 residency. Candidate must have current NYS Pharmacist license, a current Collaborative Drug Therapy Management credential, or ability to obtain within 3 months of hire. Minimum of 1 year experience in primary care, precepting students and/or residents
Preferred candidate
will hold above plus current Board of Pharmacy Specialties credential, current or working toward AAHIVP certification, certified Diabetes Educator or Board Certified-Advanced Diabetes Management, completion of PGY-2 residency in primacy care or similar experience, experience working in a community-based health care setting, experience with EMRs

Required experience: Completion of PGY-1 residency or 3 years clinical experience
Preferred education: Doctor of Pharmacy; ABPS, AAHIVP, CDE or BC-ADM
Preferred experience: Completion of PGY-2 residency primary care, ambulatory care or similar experience
This position is posted with a salary range. The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations.
