Endeavor Health Services is seeking a dedicated and compassionate Personalized Recovery Oriented Services Counselor to join our dynamic mental health services team! In this role, you will have the opportunity to make a significant impact on the lives of individuals with serious mental illness by facilitating their journey towards recovery and resilience.

Personalized Recovery Oriented Services (PROS) is a dynamic and comprehensive team-based program designed to provide integrated rehabilitation, treatment, and support services for adults (18+) facing serious mental health challenges. This person-centered, recovery-oriented model is flexible and tailored to meet the unique needs of each participant, while promoting a welcoming and supportive community environment.

At its core, the PROS model emphasizes building a strong, inclusive community for individuals at every stage of their recovery journey. We are committed to meeting individuals where they are, offering services in both structured program spaces and within the natural environments where they live, work, learn, and socialize. This approach ensures a seamless integration of care that supports each participant’s personal goals and fosters lasting recovery.

This is a full-time (37.5 hours per week) on-site position responsible for maintaining an individual caseload, providing clinical services individually and in group settings on areas of mental health, substance abuse, forensic counseling, etc. Completing assessments, documentation, and treatment plans with assigned individuals and scheduled intakes. Working collaboratively with the PROS treatment team to meet client and program needs.

Responsibilities
  • Conducting pre-admission screenings to determine appropriate level of care for the individual based on information gathered and knowledge of agency/community resources
  • Gathering information for completing appropriate strength-based treatment planning, involving both the individual and their treatment team
  • Pursuing continuing education to develop skills necessary to effectively treat high-risk populations
  • Participating in multidisciplinary treatment team meetings and communicating appropriately with referral sources
  • Conducting discharge planning and making follow-up contacts as necessary
  • Providing the people we serve with linkages to other supportive services as identified
  • Maintaining appropriate and accurate documentation within the clinical record
  • Maintaining cultural sensitivity and competence with respect to agency consumers and their families, as well as other employees and referral sources
  • Responsibility for ensuring compliance with all agency policies, procedures and governmental regulations including, but not limited to HIPAA
Qualifications
  • Master’s Degree required from a recognized college or university in a mental health or social work related field and be eligible for license within 18 months of hire (LMSW, LCSW, LMHC, LCAT, CASAC, LMFT, Licensed OT)
  • One year of direct service, post-graduate, experience preferred, but not required
  • Demonstrates a knowledge base upon which they can provide direct clinical service and advisement

Salary starting at $55,000 - $66,000/year depending on education, credential and experience.
