Job Description

The Pediatric Behavioral Health Integration Coordinator is a core member of the collaborative team, including the patient s medical director and psychiatric consultant, as well as the larger primary care team. The Coordinator is responsible for supporting and coordinating the mental health and physical health care of patients on an assigned patient caseload with the patient s medical provider, and when appropriate, other mental health providers.

Education

And Credentials

Master s degree in Social Work, Psychology, or Mental Health Counseling required. Master-level licensure candidate/trainee (e.g. LMSW) preferred. Licensed Mental Health Counselor preferred. Licensed Social Worker preferred. Master Social Worker (LMSW) within 18 months of hire required.

Experience
  • 6 months experience with screening for common mental health and/or substance abuse disorder required.
  • 6 months experience with the ability to effectively engage patients in therapeutic relationships, when appropriate, required.
  • 6 months experience with assessment and treatment planning for common mental health and/or substance abuse disorders required.
  • 6 months experience with working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders required.
  • 6 months experience with basic knowledge of psychopharmacology for common mental health disorders within appropriate scope of practice required.
