The Complex Care Manager provides time-limited interventions to restore functioning and address the symptoms of substance use and mental illness, including skill-building to help individuals identify solutions to problems that threaten recovery and care coordination services to help individuals connect with medical or other remedial services.
As part of the essential functions of this role,
The Complex Care Manager (Substance Use):
- Conducts screenings/assessments to identify an individual’s clinical and social determinants of health (SDoH) needs, choices, and their preferences for services/providers
- Assesses and addresses health and safety issues as well as barriers to care and barriers to patient’s ability to remain in a community setting
- Maintains communication between treating practitioners and other service providers to facilitate collaborative care for complex behavioral health needs
- Provides additional referral options as appropriate (i.e. primary care, testing, substance use counseling, case management, housing information, social services)
- Provides ancillary services to patients who have a high rate of hospital utilization to decrease readmission and emergency room occurrences
- Assists in collaboration among cross-system agencies, organizations, and other entities to facilitate an organized and coordinated approach to care
- Provides education and referral/linkage to needed support and services to meet the comprehensive needs of individuals/families
Qualified candidate: will have a bachelor’s degree in psychology, social work or other related field with 2 years of experience in a human services or behavioral health setting preferred OR a master’s degree with one year of experience. Must possess experience working with diverse patient populations, including but not limited to people with mental health and/or substance use concerns and individuals who are racial/ethnic minorities, women, LGBTQ+, and individuals living with HIV.

Education/Experience
:
Bachelor’s in Psychology, Social Work, or other related field with 2 years’ experience in a human services or behavioral health setting preferred
OR
Master’s degree with one (1) year of relative experience.
This position is posted with a salary range. The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations.
