A community health center in Buffalo is seeking a care coordinator to enhance patient experiences and health outcomes. Responsibilities include coordinating care, assisting patients with referrals, scheduling appointments, and collaborating with various teams. Ideal candidates have strong communication and problem-solving skills, a high school diploma (associate’s degree preferred), and experience in community health. Working hours are Monday to Friday, 8:30 a.m. to 4:30 p.m. Competitive pay at $22.25 per hour with robust benefits.
